New Client Form
We are currently accepting new client and patient applications for wellness visits only.
“Wellness” includes annual checkups, preventive care and vaccine updates.
New patients will need to have submitted all vaccine history and pertinent medical records prior to scheduling an appointment. In addition, new client forms and patient questionnaires need to be completed and sent in advance. (See forms linked below.)
Due to staffing shortages, we still can not openly receive new clients with sick patients. If you have never been here before and you have a sick pet or pet in need of surgery, please call ahead, as acceptance of these pets will be based on doctor availability.

Prior to scheduling an appointment for your pet or visiting during our walk in hours, please fill out our patient history form, linked below. If you are bringing more than one pet to be seen, please fill out one form per pet.

Health Certificate Information Form
Prior to your appointment for a domestic or international health certificate, please review and fill out the form below and bring it with you to your appointment. You can also email it back to us at occoquanah@gmail.comPlease be sure to contact us before coming in for a health certificate.
If your pet has had vaccines, tests or treatments performed at another clinic, all applicable records must be sent to us prior to making an appointment. Thank you!
