New Clients
Thank you for your interest in our hospital! Please review the following information regarding our current acceptance of new clientele.
We are currently accepting new client and patient applications for wellness visits only.
“Wellness” includes annual checkups, preventive care and vaccine updates.
If your pet is not feeling well please DO NOT fill out the applications below and instead call to speak with our staff to see if we have doctor availability to see your pet.
Due to staffing shortages, we still can not openly receive new clients with sick patients. If you are a new client and you have a sick pet or pet in need of surgery, please call ahead, as acceptance of these pets will be based on doctor availability.
New clients will need to have submitted all vaccine history and pertinent medical records prior to scheduling an appointment or visiting during our walk in hours. In addition, new client forms and patient questionnaires need to be completed and sent in advance. Please complete both the new client and patient forms below, and send your pet’s previous medical records to occoquanah@gmail.com.
Please allow our staff up to 48 hours to review your application once sent. If you have not been reached out to within 48 hours, please feel free to give us a call to ensure we received your pet’s information. If you are interested in visiting today, please give us a call to check our availability.
